APPLICATION FOR SCHOLARSHIP

The Specialist Matthew Baylis Memorial Fund awards 10 scholarships each year in the amount of $500 each to veterans and children of veterans who need assistance paying for college or a technical school.

The Specialist Matthew Baylis Memorial Fund scholarship is awarded based on the following:

• Fully completed application form and responses to questions.

• You must also include a short essay of at least 100 words addressing the theme of "What Impact has a Veteran Made in My Life?”.

Please also submit the following items with your application:

• A copy of the veteran's (your parent's or grandparent's) Honorable Discharge (DD-214) or a letter from your parent's commanding officer certifying active duty status or a copy of their Duty Orders.

 

If you cannot locate the DD-214, your may request a copy of the form at archives.gov under "Veterans' Service Records".

 

Please note the application deadline is April 1st of each calendar year.
